You’ll provide patient care, medication administration, assessments and emergency response while working alongside a supportive medical and correctional team that values consistency and accountability.

Night Shift Licensed Practical Nurse (LPN) – Signature HealthCARE – South Pittsburg Rehab & Wellness Center

Night Shift Licensed Practical Nurse (LPN) - $5k Sign On Bonus

Signature HealthCARE of South Pittsburg Rehab & Wellness Center is a 165‑bed facility that offers a comprehensive, integrated approach to health care that embraces a wide spectrum of expertise from expert inpatient rehabilitative services and short‑term respite care to skilled long‑term nursing.

What you’ll do

  • Provide direct nursing care to residents, including medication administration, wound care and skilled nursing procedures
  • Assist residents with activities of daily living and ensure comfort, dignity and safety at all times
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team
  • Complete accurate and timely clinical documentation per facility policy and procedure
  • Coordinate resident admissions, transfers and discharges according to established procedures
  • Participate in quality assurance activities, staff education and regulatory surveys as needed
  • Perform other duties as assigned by the Director of Nursing or designee

What you need to make a difference

  • Current, active LPN/LVN license in the state of practice
  • CPR/BLS certification preferred (American Heart Association or American Red Cross)
  • Minimum one (1) year of clinical nursing experience; long‑term care experience preferred
  • Experience in a supervisory or charge nurse role preferred
  • Strong clinical observation, medication administration and documentation skills
  • Knowledge of Federal and State long‑term care regulations
  • Effective verbal and written communication skills
  • Ability to work independently, prioritize tasks and exercise sound clinical judgment
  • Experience in a supervisory or charge nurse role
  • Familiarity with electronic health records (EHR) systems
  • Proficiency with Microsoft Office applications
