Director of Product Management, Care Coordination

The Director of Product Management for Care Coordination is the single accountable product owner for Veradigm’s care coordination and chronic disease management products and capabilities. The portfolio spans both the Provider and Payer sides of the business, and the role is responsible for building the care coordination product from the ground up, defining the offering, and ensuring it is scalable and repeatable from day one.

Key Responsibilities
  • Own the end‑to‑end care coordination product strategy across Provider and Payer segments, creating short‑term and long‑term roadmaps that align with corporate direction and the evolving value‑based care market.
  • Assess the current portfolio, identify highest‑value opportunities to productize, and decide what becomes a product, a service, or is retired.
  • Translate market trends, regulatory changes, and customer needs into a differentiated product vision that creates competitive advantage.
  • Partner with executive leadership to align strategy, communicate trade‑offs, and prioritize initiatives.
  • Identify white space and adjacency opportunities (risk stratification, care gap closure, transitions of care, remote monitoring, patient engagement), and develop business cases for new investment or strategic actions.
  • Lead structured discovery programs—customer interviews, site visits, advisory councils, quantitative research—to validate unmet needs and shape product direction.
  • Define and communicate outcome‑driven product requirements and prioritized backlogs for engineering and design teams.
  • Influence other product teams’ roadmaps when productization requires capabilities owned elsewhere, building cross‑team relationships.
  • Own feature prioritization and trade‑off decisions, balancing innovation, technical health, compliance, and customer‑reported issues.
  • Drive high‑quality product experience with relentless focus on usability, reliability, performance, security, and scalability.
  • Serve as the chief product evangelist, translating complex capabilities into compelling narratives for customers, prospects, sales teams, and industry audiences.
  • Collaborate with marketing to develop positioning, messaging, and content aligned with provider and payer buyer personas.
  • Ensure field readiness for new releases, equipping Sales, Customer Success, and Support with training, tools, and knowledge.
  • Support key deals, strategic accounts, and RFP responses with expert product input and participation in customer conversations.
  • Lead, mentor, and grow a team of product managers, establishing clear goals, performance expectations, and career development pathways.
  • Recruit top product management talent and build an inclusive team that reflects the diversity of Veradigm’s client base.
  • Ensure all products meet applicable regulatory and compliance standards (HIPAA, CMS rules, interoperability, quality reporting, etc.) and proactively monitor regulatory changes.
Compensation and Benefits
