Overview

Chronic Care Management Specialist - Medical Assistant at Axil Health. Work onsite in Raleigh, NC, Monday through Friday.

• Enroll patients into RPM or CCM.
• Prepare patient chart for upcoming CCM encounters.
• Make monthly check‑in calls to patients enrolled in CCM.
• Make daily compliance calls to patients enrolled in RPM.
• Communicate urgently with clinical pharmacists for medication refills or appointment requests.
• Maintain patient confidentiality and treat patients with courtesy and respect.

• Current, nationally recognized Medical Assistant (MA) certification or Licensed Practical Nurse (LPN) certification.
• High school graduate or GED equivalent.
• Professional telephone and customer service skills.
• Computer skills: data entry, Excel and use ofHR systems such as Allscripts.
• Time management, clear verbal and written communication, and team‑environment adaptability.
• Self‑motivated, compassionate, patient‑oriented clinician.
• Ability to organize competing priorities, problem‑solve, and exercise good judgment.
• Knowledge of medical terminology and calm emergency response.
• Preferred:
  Prior CCM or RPM experience; prior pharmacy experience.
