Job Summary
To provide personal care and other supportive services to patients under the supervision of a registered nurse and in accordance with the individual Plan of Treatment.

Nursing Supervisor or Home Care Coordinator
Minimum Competencies- Ability to document in a nurse's note.
- Eligible to participate in a personal care assistance program.
- Good verbal and written communication skills.
- Ability to follow instructions by written policy.
- Provide all aspects of personal care and supportive services as directed and under the supervision of a registered nurse. This includes, but is not limited to:
- Assisting patients with personal care including bathing, skin care, and mouth care.
- Assisting patients with ambulation, including ambulation with a walker.
- Assisting patients in and out of bed.
- Assisting patient with meal preparation and eating.
- Assisting patients with using the bedpan, commode, or bathroom.
- Perform household services, which will facilitate the patient's self-care at home and are necessary to prevent/postpone hospitalization.
- Immediately report any changes in the patient's condition to the patient's case manager.
- During or immediately after the visit, document all the care provided as outlined in the Certified Home Health Aide Plan of Care.
- Ensure that safety measures are being followed. This includes, but is not limited to, ensuring that there are no loose rugs in the patient's ambulation path, keeping the side rails up on the bed, if appropriate, secure the patient in the wheelchair, if appropriate.
- Complete the orientation program outlined by the organization.
- Participate in in-service education programs.
- Participate in the Interdisciplinary Treatment Meeting.
- Have an NC CNA Certification in good standing.
- TB screen.
- Hepatitis B vaccine or signed waiver.
- Covid Vaccination
