Primary Purpose of Role

The Licensed Clinical Social Worker (LCSW) provides clinical assessment, diagnosis, and therapeutic interventions to individuals, families, and groups. This role supports patients’ mental, emotional, and social well‑being while coordinating care, addressing social determinants of health, and collaborating with multidisciplinary teams to improve outcomes.

The Licensed Clinical Social Worker (LCSW) provides clinical assessment, diagnosis, and therapeutic interventions to individuals, families, and groups, supporting mental, emotional, and social well‑being while coordinating care, addressing social determinants of health, and collaborating with multidisciplinary teams to improve outcomes.

Role Logistics
  • Works on site from Axil Health Office in Raleigh, NC
  • Reports directly to Clinical Operations Director
Key Responsibilities /Critical Functions Clinical Assessment & Diagnosis
  • Conduct comprehensive biopsychosocial assessments.
  • Diagnose behavioral health conditions using DSM‑5 criteria (within scope of licensure).
  • Identify risk factors including safety concerns, crisis situations, and social needs.
  • Develop individualized treatment plans in collaboration with patients and care teams.
  • Reassess patient progress and adjust care plans as needed.
Therapeutic Intervention
  • Provide evidence‑based therapy (e.g., CBT, DBT, solution‑focused therapy, trauma‑informed care).
  • Deliver individual, family, and group counseling.
  • Support patients managing anxiety, depression, trauma, substance use, chronic illness adjustment, and life transitions.
  • Facilitate coping strategies, behavior change, and emotional regulation skills.
Care Coordination & Case Management
  • Coordinate care with physicians, psychiatrists, nurses, care managers, and community providers.
  • Connect patients to community resources (housing, food, transportation, financial assistance).
  • Advocate for patient needs across healthcare and social service systems.
  • Support transitions of care (hospital discharge, referrals, follow‑up).
Crisis Intervention & Risk Management
  • Conduct suicide risk assessments and safety planning.
  • Provide crisis counseling and stabilization.
  • Initiate appropriate emergency interventions when necessary.
  • Follow mandated reporting requirements when applicable.
Documentation & Compliance
  • Maintain timely, accurate clinical documentation in the EHR.
  • Ensure documentation supports medical necessity and payer requirements.
  • Follow ethical guidelines, professional standards, and confidentiality laws (HIPAA).
  • Participate in audits and quality reviews as needed.
Patient & Family Education
  • Educate patients and families on diagnoses, treatment approaches, and coping strategies.
  • Provide guidance on navigating healthcare and community systems.
  • Promote self‑management, resilience, and recovery‑oriented care.
Collaboration & Interdisciplinary Work
  • Participate in case conferences and care team meetings.
  • Consult with providers regarding behavioral health needs impacting medical care.
  • Support integrated care models (primary care behavioral health, population health, RPM/CCM programs).
  • Communicate treatment progress and recommendations to referring providers.
Quality Improvement & Program Support
  • Participate in quality improvement initiatives and outcome tracking.
  • Support development of behavioral health workflows and best practices.
  • Contribute to program development, group offerings, or patient education materials.
  • Stay current with clinical best practices and continuing education.
Requirements Education & Licensure
  • Master’s degree in Social Work (MSW) from an accredited program.
  • Active LCSW licensure in North Carolina.
Clinical Skills
  • Strong assessment and diagnostic abilities.
  • Knowledge of evidence‑based therapeutic modalities.
  • Experience with crisis intervention and risk assessment.
  • Understanding of social determinants of health and community resources.
Technical Skills
  • Experience with EHR systems and telehealth platforms.
  • Familiarity with documentation standards and payer requirements.
  • Ability to coordinate across healthcare and community systems.
Soft Skills
  • Empathy and strong therapeutic rapport‑building.
