Position Title: Care Manager (RN)

Work Location: Region 11 - Miami-Dade (Hialeah/ Hialeah Gardens/ Doral/Opa-Locka)

Assignment Duration: 3 Months

Work Schedule: Monday - Friday 8am-5pm

Work Arrangement: Remote (Member-facing)

Position Summary: Making calls to parents of members to collect annual assessments, monthly required update calls, a monthly face‑to‑face visit and if the visit is declined a second monthly call or a virtual visit replaces it. Calling DME companies and providers if a parent states a problem.

• Making calls to parents of members to collect annual assessments, monthly required update calls, a monthly face‑to‑face visit and if the visit is declined, a second monthly call or a virtual visit replaces it.
• Calling DME companies and providers if a parent states a problem.
• Managing difficult parents at times; good communication skills.
• Excellent documentation.
• Complete all required calls/contacts/visits or calls in place of visits, collect assessments on time, compile monthly information regarding member status into an assessment called an ECC note that is required by the end of the month.
• Zoom calls or a phone call with the PDN agency every 6 months to review the care plan agency has provided.
• Great documentation needed; education provided.

• 2 years pediatric experience required.
• Case Management.
• Field visits required.
• Computer savvy.

Education Requirement: RN

Preferred Education: BSN or ASN acceptable.
