Amae Health is a Series B, venture-backed Public Benefit Corporation that delivers specialty outpatient care for people with severe mental illness. We build and operate high-acuity, in-person clinics nationally.

We are developing proprietary clinical protocols, designing novel measurement science, and engineering an AI data science platform purpose-built for the complexity of severe mental illness. Amae Health is seeking an energetic and resourceful Community Liaison to join our Community Engagement Team. This full-time role requires a home address within the Raleigh, NC metro region.
The Opportunity:We’re actively expanding in the Raleigh and seeking a dedicated Community Liaison to support this growth. In this role, you’ll build and maintain strong relationships with community stakeholders, advocate for our innovative care model, and champion Amae’s mission to deliver compassionate, comprehensive mental health services to individuals with serious mental illness.
You’ll be deeply embedded in the community, conducting outreach to local and regional partners, including healthcare providers, hospitals, community agencies, correctional facilities, treatment centers, and other referral sources, to promote Amae’s integrated care model and develop sustainable partnerships.
As an ambassador for Amae Health, you’ll thrive in a dynamic environment, balancing tasks like engaging hospital systems and providers, coordinating complex care transitions, responding to urgent referrals, visiting emergency departments, and tracking key details to ensure seamless patient experiences. Your success will come from blending strategic relationship‑building with hands‑on coordination and problem‑solving to strengthen Amae’s community presence.
What You Will Work On:Community Outreach & Referral Partner Development - 80%
- Actively manage and grow patient referrals in your region through systematic outreach to existing provider partners
- Identify and develop new potential referral sources to grow Amae's presence and ensure a steady pipeline of patient referrals
- Coordinate rapid responses to urgent patient situations, collaborating with hospital systems, social workers, and providers to ensure timely care transitions.
- Work closely with clinical staff to ensure that referred patients are connected with the appropriate care and support
- Serve as a visible, trusted ambassador for Amae Health in your community
- Track outreach activity, referral outcomes, and pipeline in CRM to meet or exceed enrollment targets
- Develop quarterly market plans in collaboration with clinical and leadership teams
- Participate in weekly team meetings with enrollment leads and market-based staff
- Complete documentation accurately and on time
- You've built relationships professionally and know how to open doors, follow through, and keep them open over time
- You care about the people this work serves, and that shows up in how you communicate and problem-solve
- You take ownership of your market without needing constant direction, but you know when to pull in your team
- You stay organized in the field, keep your documentation current, and track what's working so you can do more of it
