RN Nurse
Job Summary This is a full‑time professional position. The role is responsible for completing the admission, initial assessment, and associated documentation for new Hospice patients. The schedule is 3‑12 hour shifts per week (11:00 am–11:00 pm), rotating weekdays, every other weekend, and one major plus one minor or three minor holidays per year.
Responsibilities- Utilize expert‑level clinical decision making to assess and develop patient plans of care for elective, urgent, and emergent admissions.
- Coordinate with patients, families, physicians, Hospice supervisors, and clinical staff when admission requirements are not met or authorized, requiring facilitation of alternative care arrangements, physician or administrative override, or delay in service.
- Collect accurate and viable patient insurance information and elevate all self‑pay patients through the appropriate financial screening process.
- Plan, provide, supervise, and document safe, competent, and compassionate hospice nursing care in accordance with physician orders, TL established policies and procedures, and regulatory guidelines.
- Participate in professional development by maintaining required competencies, identifying learning needs, and seeking appropriate assistance or educational offerings.
- Provide and document appropriate end‑of‑life information and education to patients and families relevant to each patient served.
- Participate in identifying clinical or operational performance improvement opportunities and in performance improvement activities.
- Professionally manage time, people, and technology.
- Monitor and manage the appropriate use of patients’ medical equipment, supplies, and medications.
- Collaborate with Interdisciplinary Group members to determine the patient’s level of care and coordinate the appropriate utilization of contract services.
- Actively participate in the orientation and training of new employees of all disciplines.
- Work collaboratively with IDG and other Transitions Life Care departments in achieving patient care goals.
- Obtain and maintain current knowledge of best practices related to end‑of‑life care.
- Perform other duties as assigned by the supervisor.

- Adhere to NHPCO Ethical Practices and the American Nurses Association’s Code of Ethics.
- Registered Nurse with current NC license; BSN preferred.
- Minimum 2 years of recent clinical experience. Hospice and/or home health or nursing home experience or equivalent preferred.
- Expert analysis of patient clinical information and formulation of an accurate assessment.
- Expert in medical terminology and clinical data interpretation.
- Hospice Nurse Certification (CHPN) preferred.
- Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards.
- Strong computer skills required; experience with Netsmart, Outlook, and Office 365 preferred.
- Must have reliable transportation.
- Current CPR certification.
- Must be able to travel to, enter, and function in the homes or facilities of any patient/family within the service area.
- May be required to lift and/or carry items up to 50 pounds.
- Must have sensory abilities to complete physical assessment, communicate with patient/family care providers, and complete reporting and follow‑up documentation.
- Workdays and scheduled hours with weekend and holiday commitments as outlined in the Employee Information Agreement.
