• Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.
• Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.
• Educate patients and families on health management, medications, and self-care techniques.
• Collaborate with the healthcare team to ensure the highest quality care for each patient.

We are seeking a passionate LPN for our weekday team in Orange County. This position is full‑time Monday to Friday 8am‑5pm. Licensed Practical Nurse will visit 6‑7 patients on any given shift, driving to the patients’ homes to provide skilled nursing services. The primary responsibility is the initial assessment and case development of patients in all stages of life in their homes.

Duties include assessing, interpreting, planning, implementing, and evaluating patient care according to age and diagnosis.

Must be able to check email intermittently during the week to respond to documentation requests. If you have strong assessment, skilled nursing, and documentation skills, we strongly encourage you to apply. We are 100% EMR and clinicians document in the patients’ homes with company‑issued laptops. LPNs must be okay with traveling throughout the service area.
