Legacy Homecare and Wellness is a trusted partner in delivering compassionate, personalized homecare so that our clients are able to live confidently and comfortably this role, you'll work under the supervision of a registered nurse, providing quality care to patients while assisting with ADL’s and IADL’s within patients’ home in compliance with federal, state, local government regulations and company clinical policies.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Assist patients with ADL/IADL activities which may include personal hygiene, dressing, bathing, and other daily tasks.
- Ensure quality and safe delivery of home care services to patients/clients and families/caregivers.
- Implement the plan of care formulated by the Registered Nurse.
- Use client DME (durable medical equipment) and PPE (personal protective equipment) effectively.
- Observe, document, and report activity based upon the services provided throughout the shift. Documentation may include electronic medical reporting, paper documentation, or both.
- Report any emergent situations to appropriate authorities and staff in a timely manner.

QUALIFICATIONS- High School Diploma or GED required
- At least 18 years of age
- Must be listed with no substantiated findings in the State Health Care Personnel Registry
