Description

We are seeking a full-time Licensed Practical Nurse (LPN), Behavioral Health to join our Assertive Community Treatment (ACT) team and provide compassionate, community‑based care to adults with significant mental health and substance use needs.

Your Role in Our Mission

As an LPN on our ACT Team, you will be an essential part of a multidisciplinary team providing intensive, wraparound behavioral health services. You’ll deliver medication support, clinical interventions, education, and crisis response that help individuals achieve stability, independence, and improved quality of life.

In this role, you will build meaningful relationships, help clients navigate real‑life challenges, and ensure continuity of care across community settings. You’ll work closely with psychiatrists, therapists, co‑occurring disorder specialists, and other mental health professionals to support truly holistic treatment.

This full-time LPN role follows a Monday through Friday schedule with rotating on‑call responsibilities
. Travel within the community is required.

Hourly Range: up to $30 per hour (based on experience)

What You’ll Do
Medication & Clinical Support
  • Administer psychotropic medications and monitor symptoms and health status.
  • Document all care and observations using the Welligent EMR platform.
Care Coordination
  • Collaborate with the multidisciplinary ACT team in developing individualized treatment plans.
  • Participate in daily team meetings to ensure coordinated, client‑centered support.
Crisis Response
  • Provide crisis intervention services as part of a rotational on‑call schedule.
Community-Based Support
  • Deliver nursing care in homes and community settings.
  • Advocate for client choice, educate on wellness and health management, and connect individuals with needed resources.
What We’re Looking For

To thrive as an ACT Team LPN, you should be adaptable, compassionate, and comfortable supporting clients in community environments.

Required Qualifications:

  • Active, unrestricted LPN license in North Carolina
  • Experience in mental health or substance use services (preferred)
  • Ability to work in community settings
  • Valid driver’s license, vehicle insurance, and reliable transportation
  • Strong communication, documentation, and relationship-building skills
