About the Role

The Carolina Outreach team is looking for a compassionate, dedicated, and skilled Psychiatric Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this pivotal role, you’ll play a hands‑on part in transforming lives by delivering critical, community‑based mental health services to individuals with complex needs.

Each day, you’ll bring your clinical skills to life by managing medication monitoring, delivering psychopharmacologic care within your scope, and coordinating services that help clients thrive in their communities. You’ll respond to crises, support transitions into and out of care, and collaborate on individualized treatment plans. With your help, clients won’t just navigate the system—they’ll make meaningful progress toward stability and recovery.

  • Assist in creating transition plans when clients move in and out of services.
