Full-Time | Nights (7:00 PM – 7:30 AM)Schedule: 3 twelve-hour shifts per week | Every other weekend rotation (2 weekends every 6 weeks) | One winter holiday and one summer holiday annually
Make a Difference When It Matters MostTransitions Life Care is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Hospice Home team on the night shift. In this role, you will provide hands‑on personal care and support to hospice patients while helping ensure their comfort, dignity, and quality of life. You will work closely with nurses and the interdisciplinary team in a supportive, mission‑driven environment focused on exceptional end‑of‑life care.

What You'll Do- Provide safe, compassionate personal care to patients in accordance with their individualized plan of care.
- Perform the full range of Certified Nursing Assistant duties.
- Complete accurate, thorough, and timely documentation of patient care and assigned responsibilities.
- Observe and report changes in patient condition and communicate concerns regarding patients or families to nursing staff promptly.
- Participate in interdisciplinary team meetings and care planning as appropriate.
- Assist in maintaining an organized supply room, monitoring inventory, stocking supplies, and ordering materials as directed.
- Support daily Hospice Home operations through tasks such as:
- Patient laundry
- Food preparation
- Cleaning patient rooms following discharge
- Cleaning durable medical equipment (DME)
- Light housekeeping in family and common areas
- Collaborate with the interdisciplinary group (IDG) and other Transitions Life Care departments to achieve patient care goals.
- Participate in performance improvement initiatives and quality care activities.
- Maintain patient confidentiality and uphold all privacy standards.
- Perform other duties as assigned by nursing leadership.
- Provide quality care to patients across the lifespan, from pediatric through geriatric populations.
- Demonstrate compassion, professionalism, empathy, and respect in all interactions.
- Foster a positive, collaborative team environment.
- Follow all Transitions Life Care policies, procedures, and organizational guidelines.
- Deliver care in accordance with ACHC Standards and Medicare Conditions of Participation, as applicable.
- Maintain compliance with all state and federal regulations and professional standards.
- Communicate concerns, safety issues, and potential risks promptly to leadership.
- Current North Carolina Nurse Aide I certification and active listing on the NC Nurse Aide Registry.
- Current CPR certification.
- Minimum of one (1) year of experience as a Nurse Aide.
- Ability to perform CNA duties skillfully and document care accurately, timely, and legibly.
- Reliable transportation.
- Ability to work the required night‑shift schedule.
- Ability to lift and/or carry up to 50 pounds.
- Sensory and communication abilities necessary to assess patient needs, interact with patients and families, and complete required documentation.
- Experience in hospice, skilled nursing, long‑term care, hospital, or other healthcare settings.
