Position: Field LPN, Full-time: Monday - Friday 8 am to 5 pm

The schedule for this role is Monday - Friday, 8 am to 5 pm. This is a full‑time professional position.

Job Details

The Licensed Practical Nurse (LPN) will provide routine and PRN visits for assigned patients under the supervision of the assigned RNCM and/or IDG Team Leader. The LPN may provide continuous nursing care during short‑term crisis periods on a rotating basis and offer phone support to patients and caregivers for their assigned team.

Responsibilities
  • Provide and document safe, competent, compassionate hospice nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by the physician orders and the hospice plan of care. Patient response to care is reported to a registered nurse for evaluation, intervention, and modification of the plan of care.
  • Provide and document telephone support to patients, families, physicians and partners to assist in immediate needs that may be met at a distance, including physician communication, obtaining orders, follow up on changes in patient condition and other tasks within the LPN scope of practice. LPNs will seek guidance for any situation that requires RN evaluation and modification of the established POC.
  • Review supplemental orders, chart audits and other office duties as assigned.
  • Maintain nursing competencies and attend educational offerings for professional development.
  • Provide and document appropriate end‑of‑life information and education to patients and families in compliance with CMS Conditions of Participation.
  • Perform facility chart audits as assigned.
  • Manage time, people, and technology professionally.
  • Monitor and manage appropriate use of patients’ medical equipment, supplies, and medications.
  • Collaborate with IDG members to determine the patient’s level of care and coordinate contract services.
  • Identify clinical and operational performance improvement opportunities and assist in improvement activities.
  • Participate in orientation and training of new employees of all disciplines.
  • Work collaboratively with IDG and other Transitions Life Care departments to achieve patient care goals.
  • Perform other duties as assigned by supervisor.

Requirements
  • Licensed Practical Nurse with current North Carolina license.
  • Minimum 2 years of recent clinical experience; hospice and/or home health or nursing home experience or equivalent preferred.
  • Hospice Nurse Certification preferred.
  • Knowledge of Medicare regulations, licensing laws, and accreditation standards.
  • Competent computer skills.
  • Reliable transportation.
  • Current CPR certification.
  • Ability to travel to, enter, and function in homes/facilities of any patient or family in the service area to assess and provide care.
  • Ability to lift or carry items up to 50 pounds if required.
  • Proper sensory abilities to complete physical assessments, communicate with patients, families, and physicians, and complete reporting and follow‑up documentation. Workdays and scheduled hours include weekend and holiday commitments as outlined in the Employee Information Agreement.

  • Potential exposure to communicable diseases or bodily fluids.
