Heartland Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager, delivering skilled nursing care in patients' homes or other care settings. You will assess and update plans of care, coordinate with the IDG, educate patients and families, and ensure timely documentation and compliance with regulations.
The role requires NC RN licensure, CPR, and at least 1 year of RN experience, preferably in hospice or home health.
