LPN-LVN Home Health

Protouch Staffing seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide high-quality, in-home care to patients in Raleigh and the surrounding area. This role involves working autonomously to manage patient care plans, monitor health status, and educate patients and their families. The ideal candidate will be a critical thinker with excellent communication and time management skills who is dedicated to providing personalized care that promotes patient well-being and independence.

Responsibilities
  • Provide Direct Patient Care:
    Administer medications, treatments, and therapies as prescribed by the physician. This includes wound care, dressing changes, and monitoring vital signs.
  • Patient Assessment:
    Conduct initial and ongoing assessments of patient health, documenting all observations and changes in condition.
  • Patient and Family

    Education:

    Educate patients and their families on disease management, self-care techniques, medication administration, and other care needs.
  • Documentation and Reporting:
    Maintain accurate and complete medical records. Report any significant changes in patient status or concerns to the supervising Registered Nurse (RN) or physician.
  • Care Coordination:
    Collaborate with the interdisciplinary healthcare team, including RNs, physical therapists, and social workers, to ensure a comprehensive and coordinated approach to patient care.
  • Adhere to Protocols:
    Follow all established policies, procedures, and safety protocols, including those related to infection control and handling of medical equipment.
  • Travel to Patient Homes:
    Safely and reliably travel to patient residences within the designated service area.
Qualifications
  • Education:

    Graduate of an accredited Licensed Practical Nursing or Vocational Nursing program.
  • Licensure:
    Current and valid LPN or LVN license in good standing for the state of North Carolina.
  • Certifications:

    Current Basic Life Support (BLS) certification.
  • Experience:

    Previous experience in a clinical setting is required, with at least one to two years of home health experience preferred.
  • Skills:

    Strong communication, interpersonal, and critical thinking skills. Must be able to work independently and manage time effectively. Knowledge of home health software (e.g., Homecare Homebase) is a plus.
  • Physical Demands:
    Ability to perform physical tasks such as lifting and moving patients and equipment.
  • Other:
    Reliable transportation, valid driver's license, and proof of auto insurance are required.

HCHB, Oasis, and HH experience required

Skills

Homecare Homebase Software Experience
- Required

Current Nursing License in the Practicing State

Valid Drivers License

Current CPR Certification

Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Current driver's license

Must have reliable transportation and insurance

1-2 years experience as a LPN-LVN in a clinical care setting

1-2 Years Home Health Experience
- Required.

Education

Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing

Compensation Details

Gross Pay (weekly): $1900.0

Taxable Pay (weekly): 600.0

Non-taxable Pay (weekly): 1300.0
