Position Summary
Amplify Health is staffing for an additional Family Nurse Practitioner. The Nurse Practitioner (NP) will provide comprehensive, patient-centered primary care services to individuals and families across the lifespan within a community health center setting. The NP will diagnose, treat, and manage acute and chronic medical conditions, deliver preventive care, and promote health equity for underserved populations in alignment with the organization’s mission.
Essential Duties and Responsibilities
- Conduct thorough patient assessments, including medical history, physical examination, and diagnostic tests.
- Diagnose and treat various medical conditions and illnesses.
- Prescribe medications and monitor their effectiveness.
- Developing and implementing treatment plans.
- Collaborating with other healthcare professionals to ensure the best possible patient care.
- Maintaining accurate and up-to-date medical records.
- Displays sensitivity to the populations served.
Provide care to diverse populations, including uninsured, underinsured, and medically underserved patients. - Educate patients on disease prevention, medication adherence, and lifestyle modifications.
Coordinate care with physicians, nurses, behavioral health providers, care managers, and specialists.
Participate in team huddles, case conferences, and clinical meetings.
Minimum Qualifications
- Nurse Practitioner degree from an accredited institution.
- Current North Carolina NP license.
- Completed residency training (REQUIRED)
- At least one year of experience in a primary care setting functioning at the full extent of training and licensure.
