Schedule:Saturday, Sunday and Monday 7:00am-7:30pm; one major and one minor or three minor holidays per year
This position is primarily conducted in the field, providing care to patients wherever they call home, including private residences, assisted living communities, skilled nursing facilities, and other care settings throughout our service area.
Unlike a traditional hospital or clinic-based nursing position, this role requires daily travel between patient locations, independent clinical decision-making, and the ability to build meaningful relationships with patients and families in their own environments.

We're seeking an experienced Registered Nurse who can confidently provide field visits, support admissions, manage urgent patient needs, and step into the Case Manager role when census demands or staffing coverage requires it.

What You'll Do Deliver Expert Hospice Nursing Care- Conduct scheduled and PRN hospice visits throughout the community
- Assess patient needs and provide skilled nursing care aligned with the hospice plan of care
- Monitor patient conditions and implement appropriate interventions to maximize comfort and quality of life
- Deliver compassionate support to patients and families navigating serious illness and end-of-life care
- Utilize advanced clinical judgment to assess patients and develop individualized care plans
- Conduct elective, urgent, and emergent admissions
- Analyze complex clinical information to determine appropriate levels of care and interventions
- Initiate Crisis Care in accordance with agency protocols and Medicare regulations
- Provide coverage for RN Case Managers during PTO, leaves of absence, and periods of increased census
- Function as the RN Case Manager when assigned
- Coordinate all disciplines involved in a patient's care plan
- Delegate and supervise LPNs and Certified Nursing Assistants in accordance with regulatory requirements
- Work closely with physicians, social workers, chaplains, hospice aides, and community partners
- Coordinate medical equipment, medications, supplies, and contracted services
- Participate in quality improvement initiatives and clinical excellence programs
