We are seeking a Psychiatric Nurse Practitioner to provide clinical services to all clients receiving ACT team services, which includes medication prescription, administration, monitoring, and documentation; directing psychopharmacologic and medical services; service coordination, including working with the ACT team to monitor each individual’s clinical status and response to treatment; crisis assessment and intervention; symptom assessment and management; individual counseling and psychotherapy;
substance use treatment; work‑related services; activities of daily living services; social, interpersonal relationship, and leisure‑time activity services; support services or direct assistance to ensure that individuals obtain the basic necessities of daily life; and education, support, and consultation to individuals’ families and other major supports.
Key Responsibilities:
- Supervise all medical/clinical aspects of the services delivered.
- Identify appropriate evidence‑based or emerging best practices therapies to be employed by staff in delivering services.
- Consult with primary healthcare providers/other providers on the clinical aspects of the clients being served and the implications for those clients’ physical healthcare needs.
- Coordinate with medical staff of other providers, e.g. state and community inpatient facilities.
- Supervise all other medical staff on the ACT team.
- Conduct psychiatric assessments and manage medications for clients.
- Provide treatment, crisis intervention, and ongoing mental health support.
- Collaborate with the ACT team and community providers to coordinate care.
- Ensure high‑quality, evidence‑based services.
The psychiatric nurse practitioner must be a licensed nurse practitioner with experience with the population served.
