CFSP Hospital Liaison

Location: This field-based role in the Eastern Region of North Carolina enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. These roles are county field-based and require you to interact with patients, members, or providers in person four to five days per week.

How You Will Make An Impact
  • Serve as the primary point of contact for hospitals/EDs regarding youth who are boarding and awaiting placement.
  • Coordinate and maintain communication with ED staff (e.g., social work, case management, nursing leadership, security) regarding status updates, safety concerns, and barriers to discharge.
  • Coordinate internally with CM and placement teams to align on next steps, required documentation, referrals, and placement efforts.
  • Schedule, organize, and facilitate staffing meetings (internal and external), including preparing agendas, distributing meeting links, documenting decisions, and assigning follow-up tasks.
  • Provide clear, routine written updates to internal leadership and involved teams on each youth’s placement status, risks/safety concerns, and outstanding needs.
  • Track each youth’s boarding timeline, current location/status, and key placement/discharge barriers; ensure information is current and accessible to the team.
  • Support escalation processes when safety concerns, service gaps, or systemic barriers require DHHS/leadership involvement.
  • Ensure communication is accurate, timely, trauma-informed, and aligned with confidentiality and information-sharing requirements.
  • Collaborate with hospitals and internal partners to promote continuity of care and reduce avoidable delays in discharge/placement.
Work Relationships
  • External:
    Hospital ED staff, hospital social work/case management, behavioral health teams, and other involved providers.
  • Internal:
    Care managers, placement specialists, utilization management/authorization (as applicable), and leadership.

  • Skilled at facilitating meetings and driving action items to completion.

  • Strong documentation and tracking habits; attention to detail and deadlines.
  • Ability to navigate complex systems and collaborate across agencies/organizations.
Minimum Requirements

Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community based organizations; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities, And Experiences
  • Experience working with youth in crisis, ED boarding, psychiatric placement, or child welfare/juvenile services.
  • Familiarity with community-based placements and levels of care for youth.
  • Working Conditions:

    Primarily office/remote with frequent phone, email, and virtual meeting coordination; may include periodic onsite hospital visits depending on program needs.
  • Work involves time-sensitive coordination and exposure to high-acuity situations via reports from hospitals and teams.
