As a Foster Care Case Manager, you’ll play a vital role in supporting children, empowering foster families, and helping build stable, nurturing environments where youth can thrive.

This is a dynamic, relationship-driven role ideal for someone who is passionate about advocacy, collaboration, and making a meaningful impact every day.

• Serve as a trusted partner to children, foster families, and care teams—guiding them throughout the foster care journey
• Coordinate services, treatment planning, and resources to support each child’s unique needs
• Facilitate Child and Family Team Meetings and actively contribute to admission, treatment, and discharge planning

• Provide ongoing coaching, training, and hands‑on support to foster parents
• Conduct regular in‑home visits to assess strengths, address needs, and ensure a safe, nurturing environment
• Help families navigate behavioral challenges using proven models like the Teaching Family Model

• Maintain strong communication with biological families, social workers, and community partners
• Represent Children’s Hope Alliance in court hearings, treatment reviews, and external meetings
• Respond to urgent needs and provide crisis/on‑call support when it matters most

• Keep accurate, timely documentation and case records using clinical management systems
• Monitor foster home compliance with licensing standards and conduct ongoing reviews
• Review medical administration records to ensure the safety and well‑being of children

• You’re passionate about helping children and families succeed
• You build strong relationships and communicate with empathy and professionalism
• You thrive in a fast‑paced environment where every day brings new challenges and rewards
• You’re organized and detail‑oriented, with the ability to manage multiple priorities

• Bachelor’s degree in Psychology, Social Work, or a related field
• At least 2 years of experience working with children, families, or in a related human services role
• Qualified Professional (QP) designation (required)
• Ability to respond to urgent situations and participate in on‑call rotation
