Transition Case ManagerFull Time Raleigh, NC, US 1 Attachments
Salary: $40,310.40 Annually
Role SummaryThe position works directly with Alumni and CTR II participants to support employment, housing stability, financial self-sufficiency, and successful community transition. The position provides case management and resource coordination, maintains community partnerships, and assists with the management and day-to-day operations of the Forward Together Housing program.
Role Requirements and PreferencesRequired:
- Minimum of 2 years of sustained recovery and demonstrated knowledge of recovery principles.
- Valid driver license and transportation.
- Completion of a High School Diploma or equivalent (preferred.)
- One (1) year of relevant experience, or any equivalent combination of education and experience that demonstrates the ability to do the tasks required.
- Must have the ability to define problems, collect data, establish facts, draw valid conclusions and engage in ethical decision-making.
