Job Title
Primary duties include:
Conducting a comprehensive assessment of families' strengths and needs
Participating in family service planning meetings, developing and implementing service plans with measurable goals and objectives
Working with families to overcome barriers and achieve improvement in functioning
Referring, arranging and advocating for services that a family needs
Conducting home visits to families for the purpose of counseling and monitoring of services and outcomes
Utilizing social work tools such as but not limited to genograms, ecomaps, and financial worksheets
Occasional evening and/or weekend work may be required
