Job Description:

The Registered Nurse in Home Health is responsible for providing nursing care and managing patients in their homes. Duties include conducting start of cares (SOC), resumption of cares (ROC), recertifications, and daily visits.

Responsibilities:

• Start of cares (SOC) and resumption of cares (ROC)
• Complete recertifications and daily visits
• Maintain productivity expectation of a SOC and 3 RVs or 5 RVs per day
• Ensure visits are geographically manageable or utilize discretionary points for extended travel

Requirements:

• At least one year of experience in Home Health within the last two years
• BLS certification and an active RN license in California
• EMR experience with Home Care Home Base (HCHB) strongly preferred
• Two supervisory references required at submission; one must be within the last 12 months and one within three years
• A copy of the driver's license is required at submission to validate the legal name
• Availability for one weekend commitment per four-week schedule
• No employment at facilities associated with Common Spirit, CHI, or Dignity within the past year
