Overview
BAYADA Home Health Care is seeking a Home Health Physical Therapist for a per diem opportunity in Randallstown, Maryland. The office services adult and geriatric clients on a per visit basis in territories throughout Maryland, including West Baltimore County (Reisterstown, Owings Mills, Randallstown, and Pikesville).
Per Diem requires current home health and Oasis experience. One year of prior clinical experience as a licensed Physical Therapist is required. This role involves visiting patients at home and being an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing to clients at home.
Responsibilities- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, including mobility and range of motion, transfer status, stability, gait, strength, endurance, balance, and safety assessments.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Develop and implement individualized care plans, including manual therapies, gait training, balance training, and other interventions.
- Continuously assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan as appropriate.
- Educate clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence at home.
- Accurately document observations, interventions, and evaluations pertaining to client care using the state-of-the-art touch pad tablet.
- Maryland Physical Therapist license (PT).
- Minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the AMA, as indicated by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Base pay: $57.00 – $73.00 per hour, depending on qualifications.
