Shift – Hours PRN – As Needed – 8:00am‑4:30pm

Join a growing healthcare system at Life Bridge Health’s Northwest Hospital as a Nurse Navigator LPN
. The goal of the Nurse Navigator LPN is to empower patients in the post‑acute selection process and create a seamless transition of care aligned with Life Bridge Health’s core values.

• Educate patients recommended for skilled nursing or rehabilitation on post‑acute options, including discussion of quality analytics for skilled nursing facilities.
• Apply critical thinking to anticipate barriers to discharge and collaborate comfortably with an interdisciplinary team.
• Maintain excellent verbal and written communication, focusing on customer service, with patients, families, post‑acute care facilities, and the interdisciplinary team.
• Exhibit superior organizational and time‑management skills; multi‑task, manage numerous priorities, and maintain productivity amid interruptions.
• Review all patients recommended for acute care discharge to destinations other than their home setting, focusing on those with a discharge plan to acute or sub‑acute level of care.

• Less than one year related experience.
• High School or GED.
• Licensed Practical Nurse – Current Maryland license or eligibility to obtain a Maryland license.
