RN
- Case Manager
RN Case Manager - focus on ED and medical inpatient coverage. 40 hours/week, 5 hour shifts 08:30 - 17:00, every 4th weekend. Candidate is expected to be able to hit the ground running with about 3 days of orientation. Patient ratio ideally assigned to 1 unit (20 beds) in partnership (CM & SW). Staffing dependent, may be 1.5-2 units in partnership during crisis staffing.
Experience:
minimally 2 years hospital inpatient med-surg/critical care discharge planning; no therapists. MD license must be in hand.
