Human Service Associate/Support Planner (Randallstown, MD)

Human Service Associate/Support Planner needed for our government client in Randallstown, MD. The Human Service Associate/Support Planner will provide on-going intense case management services to 45+ clients who are older or disabled in a private home or assisted living setting. One year contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $20.25 per hour.

Essential Duties
  • Perform in-home assessments to evaluate the health, cognitive, functional, and behavioral needs of participants, including reviewing general diagnosis and treatment, activities of daily living, and independent activities of daily living.
  • Coordinate contacts to schedule in-home assessments.
  • Conduct face-to-face interviews with participants, responsible parties, or service providers.
  • Review and monitor medical documentation such as urgent care visits, hospital discharge plans, nurse monitor reports, InterRAI assessments, adult day care updates, and DDA program reports.
  • Develop person‑centered plans of service (POS) and recommend revisions when necessary.
  • Monitor implementation of services, community settings compliance, and OTP device utilization.
  • Check ISAS and ensure timely completion of nurse monitor visits.
  • Record changes in participants’ health and well‑being and evaluate the effectiveness of waiver and non‑waiver services.
  • Advocate for clients with physicians, insurance companies, and other agencies.
  • Document all case activity, including date, time, type of services and mode of service, for Medicaid billable services.
  • Submit reportable events and incident reports as required.
  • Investigate client complaints regarding quality of service.
  • Attend annual and quarterly assessments and required monthly contacts for each client.
Qualifications
  • At least two years of human service case management experience working with the elderly or disabled population.
  • Bachelor’s degree in Human Services, Psychology, Public Health, Social or Behavioral Science.
  • Ability to learn and utilize the Maryland LTSS computer system.
  • Proficient in Microsoft Word, Excel and Access.
  • Good written and verbal communication skills, including the ability to read and interpret regulations.
  • Knowledge of medical conditions and terminology.
  • Valid driver’s license, reliable transportation and a clean driving record; able to travel throughout Baltimore County.
  • Completed CPR and First Aid training.
  • Ability to work 8:30 am‑4:30 pm Monday‑Friday with a 1‑hour unpaid lunch; hybrid work schedule may be evaluated after six months.
  • Pass background check and drug testing.
