Case Mgr Social Worker-LMSW

Life Bridge Health is seeking a dedicated Case Manager Social Worker to join our Healthcare & Medical Services team. In this role, you will coordinate patient care, conduct psychosocial assessments, and develop individualized care plans to support safe transitions across the continuum of care. You will collaborate with interdisciplinary teams, connect patients and families with community resources, and advocate for patient needs.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement individualized care plans to support medical and social needs
• Coordinate services across hospitals, community resources, and social support agencies
• Advocate for patients to ensure access to benefits, housing, and behavioral health care
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize care transitions
• Provide crisis intervention, counseling, and safety planning as needed
• Educate patients and caregivers on treatment options, resources, and care plans
• Document assessments, interventions, and outcomes in the electronic health record
• Monitor progress, adjust care plans, and participate in case conferences
• Support quality improvement and population health initiatives within Life
