Position SummaryThe Medicaid Treatment/Discharge Planner is primarily responsible for coordinating Medicaid services for youth served within the 29i Health Facility. This role ensures continuity of care between residential placement, community-based providers, schools, medical professionals, and families. The Planner supports discharge planning and transition to home or community-based living while maintaining compliance with Medicaid and agency requirements.
DepartmentThe 29i Health Facility provides structured, medically supported residential care for youth. The program focuses on coordinated treatment, stabilization, and safe transitions to community-based services through collaboration with internal teams and external providers.

Pay and BenefitsStarting Pay: $19.04 – $20.00 per hour, based on education and years of relevant experience
Paid Time Off: 3 weeks PTO in the first year
Holidays: 14 paid agency holidays

PositionSpecific Duties /Responsibilities
Facilitate exchange of health information and documentation of care received in the community.
Attend community-based care meetings as needed.
Coordinate Medicaid services and support transition planning to home or community-based living.
Coordinate outpatient treatment services as part of discharge planning.
Coordinate care at community schools when services are delivered through providers.
Schedule routine health-related appointments and distribute weekly schedules to appropriate staff.
Accompany youth to healthcare appointments and communicate relevant information to Health Services staff.
Inform Health Services Team of appointment concerns or need for additional providers.
Ensure providers have appropriate billing information and communicate insurance concerns to Medical Billing.
Document all health-related appointment information in Netsmart.
Report youth behavior during appointments to supervisor and relevant staff.
Assist with supervision of ill youth in the infirmary under nursing direction.
Maintain cleanliness of infirmary and examination areas.
Administer medication under Health Services Nurse direction while maintaining certification.
Transport urine specimens to approved laboratories.
Assist with maintaining and preparing youth files for admission and discharge.
Manage inventory of health services supplies.
Monitor Health Services area during staff absence following written instructions.
Maintain vehicle safety while transporting youth and report concerns.
Maintain Therapeutic Crisis Intervention and First Aid/CPR certifications.
Report suspected child abuse or maltreatment as mandated.
Comply with Corporate Compliance Policy.
Attend required in-service trainings and supervision meetings.
Perform all other duties as assigned.
Knowledge, Skills, and AbilitiesAbility to work effectively with youth, families, staff, and community contacts from diverse backgrounds.
Ability to deal effectively with youth and families.
Ability to accept supervision and function as part of a team.
Flexibility and adaptability.
Strong written and verbal communication skills.
Proficiency with typing and computer systems, including strong grammar and writing ability.
Ability to communicate with insurance companies, medical offices, and parents.
Strong organizational skills.
Valid driver’s license, adequate auto insurance, and compliance with agency driving policies.
Ability to pass annual physical for Category 1 job classification.
Compliance with agency safety standards.
QualificationsBachelor’s degree in a human services field preferred, or
Associate’s degree with two years of experience in a related field preferred
Previous experience in child care preferred
