The role requires weekend on-call coverage from Friday 5:00 PM to Monday 8:30 AM, with responsibilities spanning assessment, care planning, symptom management and coordination with physicians and hospice staff.
Qualified candidates hold a State Certified RN license, hospice/home health experience, strong documentation and communication skills, and a valid driver’s license with willingness to travel locally
