LPN Medical Care Manager

Role: Licensed Practical Nurse (LPN) Medication Nurse responsible for providing safe medication administration, health monitoring, and accurate clinical documentation in a senior living setting.

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real time
• Monitor residents' symptoms, report adverse reactions, and support care plan updates
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and ensure compliance standards
• Transcribe physician orders and conduct Service & Health Updates (SHUs)

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred

Key Duties: Ensuring medication safety, early health change identification, and resident safety through clinical precision and regulatory adherence.
