As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:
  • Providing skilled nursing visits in patients' homes under the direction of the RN Case Manager.
  • Performing nursing procedures and treatments ordered by the physician and outlined in the plan of care.
  • Monitoring patient conditions and reporting changes to the supervising RN.
  • Reinforcing education provided by the RN to help patients and families manage their health with confidence.
  • Administering medications and treatments within the LPN scope of practice.
  • Accurately documenting all patient care and clinical observations.
  • Collaborating with physicians, nurses, therapists, caregivers, and other members of the healthcare team.
  • Helping patients maintain independence, dignity, and the highest possible quality of life.
A few must-haves for Home Health Licensed Practical Nurses:
  • Graduate of an accredited practical nursing program and active LPN license in South Dakota
  • Minimum of 1 year of LPN experience, ideally in home healthcare (preferred)
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Compassionate, dependable, and committed to providing exceptional patient care.
  • Organized and able to manage their daily schedule independently.
  • A strong communicator who enjoys building relationships with patients and families.
  • A team player who values collaboration and continuous learning.
  • Comfortable using electronic medical records and completing timely documentation.
  • Reliable transportation, current driver's license, and current automobile insurance
