Position: Home Health LPN: 1:1 Care, Flexible Hours, Growth

Interim Health Care of the Black Hills is seeking a Home Health Licensed Practical Nurse (LPN) to join our compassionate team in Rapid City, SD. You will provide skilled nursing visits in patients' homes under the direction of an RN Case Manager, helping patients stay safe, healthy, and independent in their own homes.

This role emphasizes 1:1 patient care, coordination with the clinical team, and adherence to home health regulations while delivering education and high-quality care.
