Children’s Home Society of South Dakota is seeking a Case Manager to provide in-home case management services and appropriate interventions to children and families in accordance with the organization’s mission, vision, and values. • Work-Life Balance: Monday – Friday; with ability to flex hours depending on client needs.

Case Management

• Provide case management services to children and youth, to include home visits, community navigation, family contacts/visitation, referral for services within the community, transportation, and other needs as identified through case planning process.
• Complete evidence based assessments during case plan evaluation process for child and/or caregiver.

• Must be able to provide courteous and respectful personal attention to promote individual client satisfaction and the public perception and reputation of the Agency.

• Must be able to get important points across in the process of responding to children, adult clients, and co-workers.
