Part time PMHNP - Carbondale

As a PMHNP with Jaywalker Lodge, youll have the opportunity to do more than manage symptomsyoull help men build a foundation for lasting recovery. Youll work alongside an experienced, multidisciplinary team to provide psychiatric care for clients navigating substance use, trauma, and co-occurring mental health conditions in a supportive, community-based setting. RESPONSIBILITIES

• Conduct psychiatric evaluations to include intake assessments as well as follow-up assessments (approximately 3 days onsite, 10 to 15 hours total per week)
• Create, communicate, and carry out treatment plans with the patient and patients families
• Prescribe medications when necessary
• Communicate with patients regarding diagnosis and treatment plan
• Make referrals to specialists as appropriate
• Provide services at community partnership location
• Other duties as assigned

Qualifications

• Must possess an unrestricted license to practice in the state of Colorado
• Must possess an unrestricted DEA license in the state of Colorado
• Must have a current board certification by a national certifying body as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
• Eligible for paneling on all major insurance plans and to participate in Medicare
• Excellent communication, patient relations, time management skills, and safety awareness are required
