Tru Care Management is seeking a full‑time Nurse Practitioner who wants to become an integral part of a skilled nursing facility and make a real difference in the lives of residents every day.

This is not a typical provider role. We are looking for someone who enjoys building relationships, collaborating with facility staff, and becoming a trusted clinical presence throughout the building. The ideal candidate will round daily, provide primary care services to residents, assist nursing staff with clinical concerns, and help improve overall resident outcomes.

• Serve as the primary care provider for assigned residents.
• Perform initial assessments and routine follow‑up visits.
• Manage chronic and acute medical conditions.
• Round throughout the facility daily and maintain a visible clinical presence.
• Collaborate closely with nursing staff, therapists, social workers, and administration.
• Respond to resident condition changes and provide timely interventions.
• Participate in care plan meetings and interdisciplinary discussions.
• Communicate with residents and families regarding care plans and medical concerns.
• Help reduce avoidable hospitalizations through proactive clinical management.
• Maintain accurate and compliant medical documentation.
• Support facility quality improvement and regulatory readiness initiatives.

• Active Nurse Practitioner license in the state of practice.
• Board Certified Nurse Practitioner (FNP, AGNP, ANP, or equivalent).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and relationship‑building abilities.
• Ability to work independently while collaborating effectively with facility teams.

• Experience in Skilled Nursing Facilities (SNF), Long‑Term Care (LTC), Assisted Living, or Geriatrics.
• Experience caring for medically complex patients.
• Familiarity with Point Click Care  or similar EHR systems.
