Overview
$1,500 sign-on bonus. LPN
Schedule:
Every other Friday, Saturday, Sunday 8p – 8a (Shift differential offered). Homecare with Heart offers competitive pay. Starting rate for LPN: $26 per hour depending on experience and availability.
- Administer medication in accordance with standards of practice.
- Encourage social and rehabilitative skills and promote self-care and independence.
- Perform client treatments in accordance with Plan of Care.
- Assist the consumer in learning appropriate self-care techniques.
- Prepare clinical and progress notes to document care provided.
- Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.
- Promote activities to achieve a safe and healthy environment.
- Observe aseptic technique when performing client procedures/care.
- Collaborate with other members of the clinical team to develop and improve the Plan of Care.
- Assist the client in achieving established goals in the Plan of Care developed by the RN.
- Maintain productivity according to the standards of the agency.
- Maintain license, CEUs and mandatory employment requirements.
- Participate in client care meetings, in-services, orientation and staff meetings.
- Provide service in accordance with agency’s philosophy, standards, policies and scope of practice.
- Participate in Performance Improvement and adhere to HIPAA guidelines.
- Other duties as assigned.

What You’ll Bring to the Team:- Valid State LPN License
- Valid driver’s license and use of automobile is required due to the travel involved.
- Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
- Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
- Must be able to use a computer, laptop or tablet.
- Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
