Position: Social Service Worker 3

JOB OBJECTIVES

The Family Team Meeting Coordinator reports to a Child and Adult Protective Services Supervisor. The coordinator’s primary role is to set up and facilitate the Family Team Meeting by meeting with the family, invited participants and service providers with the goal of establishing plans that support the best interest of children. The coordinator will arrange for a meeting place, invite the participants, and share information with the participants as to the focus of the meeting.

The coordinator will summarize verbally and in writing outcomes and action steps from each meeting. The Family Team Meeting Coordinator will also provide support to the CARES line.

ESSENTIAL JOB FUNCTIONS
  • Ensure the initial FTM is held as soon as possible to engage families early and support the creation of a case plan that links families to timely services and wrap‑around support.
  • Ensure full representation of family and supportive services providers.
  • Encourage full participation of all participants with honest communication and promote dignity and respect among all meeting participants.
  • Encourage participants to work together to create a realistic and achievable case plan that supports safety and permanency for children.
  • Ensure the meeting covers all areas established within a Semi‑Annual Review (SAR) report.
  • Coordinate, schedule, and facilitate all follow‑up meetings to ensure all parties are accountable to the action steps established within the meetings. (FTMs should be held at least quarterly—every 90 days—throughout the duration of a case).
  • Work in tandem with the Quality Assurance Specialist and Program Specialist to assess effectiveness of service and report outcomes related to participation and completion of action steps established through this process.
  • Receive reports of child and elder abuse and neglect through the abuse hotline, screen information, and prioritize reports to determine if further investigation is appropriate according to State mandates.
  • Monitor and participate in the evaluation of support services, including collateral contacts with schools, law enforcement agencies, health care personnel, community and Juvenile Court authorities.
  • May monitor court‑ordered supervised visits between child and family members.
  • Initiate and maintain case records, dictation, and preparation of required forms, case narratives, correspondence, and case documentation.
  • Testify in court proceedings and/or prepare and file court‑required child welfare documents, as required.
  • Participate in child welfare after‑hours C.A.R.E.S. Program, providing initial investigation and crisis intervention on after‑hours, weekend and holiday child abuse and neglect referrals.
  • Adhere to State‑required mandates and submit required documentation in required time frames.
  • Enter/retrieve data from automated computer system such as SACWIS and EDMS.
  • Perform initial assessments and investigations concerning elder abuse, elder neglect or elder exploitation, as defined by ORC 5101.62 for persons 60 years of age or older. Provide initial crisis intervention, protection and counseling to APS clients and arrange for treatment services and supportive services goals.
  • Assist clients and/or their community‑based service agency for admission to medical treatment centers, adult foster care or nursing homes.
  • Petition the county Probate Court to include testifying concerning safety and/or competency hearings when necessary.
  • Participate in various agency‑required meetings (Family Team Meetings, Prosecutor meetings, Division meetings, Unit meetings, etc.).
  • Provide training and service coordination on Independent Living Skills for youth ages 14‑17.
  • Keep current in areas of responsibility by attending skill development conferences, seminars, or State‑sponsored programs related to Federal, State laws, rules and regulations.
  • Demonstrate regular and predictable attendance.
  • Operate a motor vehicle as needed to perform essential functions.
NON‑ESSENTIAL JOB FUNCTIONS
