Licensed Practical Nurse

River Woods Exeter seeks compassionate RN/LPN nurses to provide high-quality clinical care to residents in our continuing care retirement community. In this role, you will deliver direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with an interdisciplinary team to support individualized care plans. You'll educate residents and families, maintain accurate documentation, and promote safety, dignity, and respect.

Responsibilities

• Provide direct nursing care to residents in a continuing care retirement community setting
• Administer medications, treatments, and injections according to physician orders and facility policies
• Monitor, assess, and document resident health status, reporting changes promptly to providers
• Develop, implement, and update individualized resident care plans in collaboration with the care team
• Supervise and support LPNs (for RNs), LNAs, and other support staff to ensure quality care
• Maintain accurate, timely electronic health records and required documentation
• Educate residents and families on health conditions, medications, and care plans
• Promote resident safety, infection control, and adherence to clinical best practices
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives
