Position: Registered Nurse (RN)

Language requirement:
Fluency in English and Spanish is required.

Work schedule:

7pm-7am, weekends required.

Academic requirement:
Registered nurse license required;
Bachelor of Science in Nursing preferred.

Certifications:

Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention.

Critical Action Items & Measurable Deliverables
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Provide triage services to assess patient needs and evaluate symptoms, including answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
  • Have extensive knowledge of medical documentation to generate, decipher, and correct documentation methods in accordance with state and federal guidelines.
  • Perform nursing duties and tasks relevant to this position in compliance with all regulatory authorities and program protocols, including active and ongoing nursing assessments, charting, scheduling appointments, routine observations, isolation observations, wellness checks, immunizations, medication administration, and internal reports as needed.
  • Ensure patient safety by assessing and monitoring health conditions, assisting with procedures, operating medical equipment, administering vaccines, medications, and treatments ordered by a healthcare provider, monitoring for side effects and reactions, and providing teaching to patients and staff.
  • Arrange and coordinate appropriate health services, including urgent, emergent, and preventative health services for children in care.
  • Conduct health assessments based on established protocols and accurately document medical findings, validate systems of record for required facility documentation, and report to meet agency, federal, and state guidelines within required time frames.
  • Supervise, direct, and delegate when appropriate to execute medical services reflected in healthcare plans and treatment orders, ensuring all treatment orders directed by a prescribing healthcare professional are carried out and monitoring the patient for improvement.
  • Gather, collect, and clarify patients’ current condition and past medical history.
  • Communicate with pharmacies to order prescription medications, verify medication deliveries for accuracy and compliance with orders, and remediate any discrepancies.
  • Maintain adequate over‑the‑counter medication supplies for each service population facility and arrange proper disposal of unused and expired medications.
  • Maintain documentation of controlled drugs, including up‑to‑date reconciliation of all scheduled drugs used in the facility through the facility’s protocols and procedures.
  • Administer medication as ordered by a prescribing provider and complete accurate, timely documentation in electronic medical records and the Medication Administration Record.
  • Effectively communicate with the Certified Nurse Practitioner and Lead Medical Coordinator as needed for reporting findings concerning a patient’s health and well‑being.
  • Communicate with Lead Medical Coordinator and other medical staff to devise individual plans of care and act as a nurse support to other medical staff.
  • Implement Compass Connections safety protocols, including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and personally identifiable information protocols and procedures.
  • Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.
