Admissions Coordinator

We are currently seeking an experienced LPN to join our team as an Admissions Coordinator at Foxwood Springs. As an Admissions Coordinator, you will be responsible for managing the admission process of new residents, ensuring that they receive the highest level of care possible.

• Managing the admission process, including coordinating with families, physicians, and other healthcare providers
• Conducting initial assessments of potential residents to determine eligibility for admission
• Coordinating with other departments to ensure that the resident's needs are met
• Providing support and guidance to families throughout the admission process
• Maintaining accurate records and documentation

• Active LPN license with minimum five years of SNF experience and critical skill set experience required.
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time-management skills
• Knowledge of healthcare regulations and laws
