Position Summary
Independent Optometrist leasing space inside Walmart stores. Lead patient care for eye and vision, plus general health and well‑being. As a lease holder you control practice schedule, hours, patient booking, fee setting, and keep 100% of exam/professional fees.
At fair market value, you may opt into equipment and instrumentation, as well as administrative and/or Opt‑Tech support services (where state permits).
Responsibilities- Establish days of practice
- Establish hours of operation
- Control patient schedule
- Set fee schedule
- Doctor of Optometry (OD) from an accredited optometry school and valid state optometry license
- Ophthalmologist (MD or DO) from an accredited school and valid state ophthalmology license
- General Liability Certificate
- Professional Liability Insurance Certificate
- W‑9
- ME & VA license agreements available through Gumberg Asset Management Company
