Family Service Association is seeking a part‑time (21 hours per week) Site Coordinator for our Countryside Adult Day Health Program in Raynham, MA. The Site Coordinator assists the Program Coordinator with management of day‑to‑day operations, provides case management services, and monitors the mental health status of program participants.
The hourly rate is $21 and the shift is Monday through Friday, 10:00 a.m. – 2:00 p.m.
$500 sign on bonus
Duties and Responsibilities- Provide case management services
- Develop and lead groups for participants
- Assist the program coordinator with referral, admission, and discharge processes
- Write quarterly progress notes
- Inform participants and their families/caregivers of available community services and make referrals as indicated
- Attend monthly supervisory meetings with the Program Coordinator (Bi‑weekly with LICSW if not licensed)
- Assist, as necessary, in the delivery of other required program services
- Participate in the bi‑annual or quarterly review of each participant’s care plan and assist with development of care plans
- Assist Program Coordinator with implementation of policies and procedures
- Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure
- Provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator
- Bachelor’s Degree in Health or Human Services or a related field required
- At least one (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required
- Significant work experience with the elderly and/or physically/mentally challenged adults in a community setting preferred
Must be able to successfully complete a MA State CORI check, CPS check and company‑paid pre‑employment physical.
