• Provides professional nursing care.
• If an LPN/RN works under the direction of a Registered Nurse.
• Communicates appropriate status/condition changes to the Medicaid Manager per agency guidelines.
• Manages/supervises a team of Home Maker Aides, CNAs, and In-Home Aides to provide effective and quality home care services.
• Participates in human resources management to achieve quality service delivery and positive employee relations.
• Promotes personal safety and safe environment for client and coworkers.
• Performs job duties in accordance with agency policies, procedures, and professional and community standards.
• Provides on-call backup, as needed.
• Develops working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to clients.
• Promotes professional safety and safe environment for clients and co-workers.
• Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements.
• Assesses safety of environment and takes the initiative to help prevent accidents and promote safety.
• Recognizes and responds appropriately to potentially unsafe situations.
• Performs other duties as assigned.
