In this part‑time role, you'll play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
About the RoleAs a Social Worker/ Social Worker, you'll have the opportunity to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. Our experienced clinical team operates seamlessly with your facility's processes and protocols to ensure minimal disruption to your current operations.
Key ResponsibilitiesVisit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short‑term, solution‑focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health‑related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long‑term care settings
Strong communication, documentation, and relationship‑building skills

Regular supervision and collaboration with psychiatric providers

Flat salary: $16,000 - $18,000 annually
Pick the day and hours that work for you! Approximately 10 hours per week during Monday-Friday, within regular 8-5 hours
