RN Case Manager

Starting Salary: $93,600

Responsibilities- Completing an initial assessment of patient and family to determine hospice care needs.
- Regularly re‑evaluating the patient’s nursing needs.
- Using assessment data to determine care plan needs.
- Collaborating with the entire healthcare team, patient, and family to direct work toward continuous and well‑coordinated care.
- Counseling the patient and family in meeting end‑of‑life needs.
- Initiating appropriate preventative and palliative nursing procedures.
- Communicating with the physician regarding patient needs and reporting any changes in the patient’s condition.
- Supervising the Hospice Aides.

- Associate's Degree in Nursing.
- State Certified RN license required.
- 2 years’ experience as a professional nurse; hospice experience preferred.
- Valid driver’s license and reliable transportation.
- Willingness to travel.
- Strong verbal and written communication skills.
