We are seeking a dedicated and compassionate PCA to join our team at NuChoice Health Partners, a leading home care agency serving the Northwest of Pennsylvania. As a PCA with our organization, you will have the opportunity to make a meaningful difference in the lives of our patients while providing high-quality care in the comfort of their own homes.

As a PCA
, you will be responsible for providing companion care, meal preparation, medication reminders, and personal hygiene assistance to our patients. You will work closely with our interdisciplinary team to ensure that our patients receive the best possible care, with a focus on dignity, respect, and compassion.

• Provide companion care, including socialization and emotional support to patients
• Prepare meals and snacks according to patient's dietary needs and preferences
• Assist with medication reminders and administration, as needed
• Assist with personal hygiene, including bathing, dressing, and grooming
• Maintain accurate and timely documentation of patient care and services provided

• Prior experience as a PCA or in a similar healthcare setting
• Ability to lift, transfer, and ambulate patients as needed
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable transportation and a valid driver's license (if applicable)
