Job Description Summary

The Physical Therapist I is responsible for the evaluation, treatment, and discharge planning of all patients referred for their given services. The Physical Therapist I is an integral healthcare provider accountable for assisting the patient to their highest level of physical, cognitive, linguistic, and swallowing function. The Physical Therapist I will uphold the mission, vision, and values of Jefferson Health. Works in conjunction with other health care professionals to coordinate treatment plans and care.

• Provide therapy evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, outpatient settings as appropriate.
• Participate in student education out of part‑time e.g. ICE, OT level 1 or a full‑time student. Provide education and teaching commensurate with novice clinician.
• Plan, organize, and deliver treatment programs that help clients/patients recover and rehabilitate.
• Utilize tools and techniques to administer safe therapeutic interventions and training for patients and families.
• Record and document clients/patients' health progress information for evaluation, all therapeutic interventions including daily treatments, progress notes, evaluations and discharge summaries.
• Interact with co‑workers, visitors, and other staff consistent with the values of Jefferson.
• Performs other duties as assigned.

• Bachelor’s, Master’s, or Doctorate degree from an accredited Physical Therapy Program.
• Less than 1 year 0‑2 years of clinical practice required.
• Licensed Physical Therapist in the State of Pennsylvania.
• BLS – Basic Life Support – American Heart Association.

Lift and carry 40‑50 lbs. Examples:
Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient’s weight. Frequent to continuous standing/walking. Patient transporters can walk 8‑10 miles per shift. Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
