10:30 AM – 6:00 PM (7.5-hour shifts)

30 hours per week

Pay: Competitive hourly rate based on experience and qualifications.

Penn Home Health Care is seeking a compassionate, dependable, and professional Direct Care Worker/Caregiver to provide one-on-one, in-home support to an older adult residing at Luther House Senior Independent Living in West Grove, PA.

This is an excellent opportunity for a caregiver looking for a consistent daytime schedule with one participant in a clean, smoke-free apartment.

• Assist with bathing, dressing, grooming, and personal hygiene

• Provide toileting and incontinence care as needed

• Assist with ambulation and mobility using a rolling walker

• Provide supervision to help reduce fall risk

• Monitor blood pressure using an electronic blood pressure monitor

• Prepare light meals and snacks

• Perform light housekeeping related to the participant's care

• Laundry and linen changes

• Provide medication reminders

• Assist with approved household tasks as outlined in the care plan

• Complete Electronic Visit Verification (EVV) and required documentation

• Report changes in condition, safety concerns, or incidents to the agency promptly

• Independent senior living apartment

• Lives alone

• No pets

• Non-smoking home

• Uses a rolling walker

• No Hoyer lift required

• Light physical assistance

• Experience providing personal care preferred

• Direct Care Worker, Caregiver, Personal Care Aide (PCA), Home Health Aide (HHA), or Certified Nursing Assistant (CNA) experience strongly preferred

• Compassionate, dependable, and professional

• Ability to work independently while following a care plan

• Reliable transportation and dependable attendance

• Ability to work independently in a participant's home while following an individualized care plan

• Must meet all Pennsylvania home care employment requirements

• Consistent weekly schedule
